To request services from the SCIDB Project, please fill out one of the forms below (and include the census form if necessary):

For Professionals: Request for Technical Assistance Services

For Parents or Caregivers: Interest Survey

Remember that the child must already be registered on the SC Deaf-Blind Census to qualify for services. If the child is not already on the Census, please fill out as much of the Census Form as possible, and return it with your Request for Technical Assistance Services or Interest Survey form. If you do not have all of the information on the census form, please submit what you can. Our team will work with you to obtain the rest of the information.
